Appointments

If you are ready to make an appointment, you may use the form below to request it. The first appointment will last 75-minutes and will be used to get a detailed history and description of your problem. You will receive a confirmation phone call within 2 business days of your request. At that point, you will be directed to download, print and fill out some forms that are available under the “Forms” tab.
